Lung disease due to inhalation of antifungal agent for home use

A 73-year-old man presented to our hospital on the 7th day of illness with respiratory symptoms that initially developed 3 days after inhalation of an antifungal cleaning agent for home use. He was treated with invasive positive pressure ventilation and corticosteroid therapy, which improved his respiratory condition, and was extubated on the 11th day of illness, and discharged on the 29th day of illness. However, his cough and fever relapsed after cessation of corticosteroids. He underwent thoracoscopic lung biopsy that showed fibrinous exudation and organization with fibrous changes, which indicated usual interstitial pneumonia. We diagnosed him with lung disease due to antifungal agent inhalation, and acute exacerbation of chronic interstitial pneumonia.
