Case Report

Sarcoidosis induced by IFN and ribavirin combination therapy for chronic hepatitis C

ABSTRACT

A 58-year-old man received combination therapy with PEG-interferon alfa-2b and ribavirin for treatment of chronic hepatitis C. Sixteen months later, he complained of general fatigue and dyspnea. Chest CT film revealed mediastinal and bilateral hilar lymphadenopathy, and he was referred to respiratory medicine. An ophthalmologic examination revealed bilateral uveitis. Because the specimens of transbronchial biopsy were not diagnostic, mediastinoscopic surgery was performed. The pathological diagnosis of the resected lymph node was sarcoidosis. After oral prednisolone therapy, the symptoms showed apparent improvement. In sarcoidosis, a predominance of Th1 immune response is well known. It is possible that interferon-alpha may trigger the Th1 reaction that takes place in sarcoidosis.
