Case Report

A case of acute eosinophilic pneumonia probably induced by daptomycin

ABSTRACT

Here we report the first case in Japan of acute eosinophilic pneumonia that was probably induced by daptomycin. A 61-year-old man had been hospitalized for debridement of a diabetic foot ulcer and was treated by daptomycin because methicillin-resistant Staphylococcus aureus was detected in his wound. Fourteen days after daptomycin treatment was initiated, he developed a high fever, cough, and dyspnea. Chest X-ray and chest CT showed bilateral ground-glass opacities and minimal pleural effusion. Laboratory examinations showed a marked increase in the percentage of eosinophils in peripheral blood and bronchoalveolar lavage fluid. With the diagnosis of daptomysin-induced acute eosinophilic pneumonia, daptomycin treatment was discontinued, and he was treated with prednisolone. Immediately after initiation of the therapy, clinical symptoms improved, and chest X-rays became clear. To the best of our knowledge, this is the first report of acute eosinophilic pneumonia probably induced by daptomycin in Japan.
