ABSTRACT

A 57-year-old woman, who for more than 10 years had suffered from rhinitis and nasal congestion, was admitted because of dyspnea and severe cough. A blood test revealed eosinophilia; a chest CT scan showed diffuse centrilobular nodules; and bronchoalveolar lavage confirmed marked eosinophilia. We performed transbronchial lung biopsy (TBLB) for definitive diagnosis. The specimens showed diffuse eosinophilic infiltration in the bronchiolar walls and eosinophil accumulation in some alveoli. Eosinophilic bronchiolitis was diagnosed based on these findings. The symptoms and blood eosinophilic counts were markedly improved after peroral administration of prednisolone, and CT findings also showed improvement. This case with eosinophilic bronchiolitis might be a categorized as having airway eosinophilic inflammation, reported previously.
