Tuberculous pleurisy patients who underwent thoracoscopy under local anesthesia in our hospital: Usefulness of tissue cultures

ABSTRACT

Thoracoscopy under local anesthesia was performed in our hospital on 88 patients with exudative pleural effusion from March 2008 through July 2013. Eighteen of these patients were diagnosed with tuberculous pleurisy, and cases that went to the submission of tissue culture totaled 14. In two of them, the pleural effusion occurred while they were undergoing treatment with anti-TNF-α inhibitors. All patients showed histopathological signs of caseating granulomas. Auramine or Ziehl-Neelsen staining was positive in 42.9% of cases. Mycobacterium culture was positive in 23.1% of pleural effusion cultures; however, it was higher for tissue cultures (64.3%). Under local anesthesia, thoracoscopy is valuable for the diagnosis of tuberculous pleurisy, and there is some possibility of its being useful to search for the presence of resistant tuberculosis.
