ABSTRACT

A 49-year-old woman was admitted to our hospital because of dyspnea. The chest X-ray revealed bilateral diffuse reticular shadows and an infiltrative shadow in the left lower lung lobe. Pathological finding of the left lower lung obtained by transbronchial lung biopsy showed adenocarcinoma in the airway interstitium, alveolar space, and lymphatic duct. Furthermore, specimens from the gastric body obtained by upper gastrointestinal endoscopy showed tubular adenocarcinoma. Taken together, the patient was diagnosed as advanced gastric cancer with lymphangiosis carcinomatosa of the lung. We speculated that gastric cancer metastasized to the lung via the diaphragm because a chest radiograph demonstrated that infiltrative shadow of the left lower lung directly involved the ipsilateral diaphragm.
