Case Report

X-ray fluoroscopy analysis of a transbronchial lung biopsy specimen for diagnosis of hard-metal lung disease

ABSTRACT

The patient was a 58-year-old female with a 3-year history of cough that was nonresponsive to antitussive drugs. She consulted our hospital for further evaluation and management. Her work had involved sanding drill ends with a wheel for the past 15 years. Thoracic computed tomography scan revealed diffuse centrilobular particle-like shadows. Furthermore, bronchoscopy with transbronchial lung biopsy (TBLB) was performed, and X-ray fluoroscopy demonstrated the presence of tungsten in the TBLB specimen. A diagnosis of hard-metal lung disease was made based on her occupational history and the results of X-ray fluoroscopy analysis. Although a TBLB specimen is markedly smaller than a thoracoscopy-guided lung biopsy specimen, diagnosis of this disease was possible by X-ray fluoroscopy analysis.
