Case Report

Pulmonary nocardiosis resulting from Nocardia abscessus in a patient with bronchiectasis

ABSTRACT

A 66-year-old woman with bronchiectasis, who complained of fever and bloody sputum, was found by chest CT to have multiple cavity shadows in the right-lower lung lobe and also small centrilobular nodules in the bilateral lung fields on bronchiectatic findings. Nocardia was detected three times in the sputum culture test, diagnosed with pulmonary nocardiosis. A switch to the sulfamethoxazole-trimethoprim from the initial treatment of meropenem showed the improvement of symptoms, blood inflammatory data, and chest imaging findings. Nocardia abscessus was identified by 16S rRNA gene analysis. In the future, N. abscessus also needs to be regarded as a pathogen of pulmonary nocardiosis in Japan.
