Case Report

A case of Corynebacterium durum pneumonia which should be differentiated from pulmonary nocardiosis

ABSTRACT

A 60-year-old male was admitted to our hospital because of high fever and falling. Chest CT demonstrated multiple cavities and nodules in the left lung. We found gram-positive filamentous bacillus in his purulent sputum. This bacteria was also stained with Kinyoun stain. We first suspected pulmonary nocardiosis; however, the Nocardia genus was not detected in the culture of sputum. The 16S rRNA sequence analysis revealed the presence of Corynebacterium durum. The patient was empirically treated with piperacillin/tazobactam, followed by penicillin G and oral-administering amoxicillin after the species and sensitivities were proved. He became better and was transferred to a different hospital for rehabilitation. Although C. durum pneumonia is a rare respiratory infection, it should be considered in the differential diagnosis of pulmonary nocardiosis.
