ABSTRACT

A 78-year-old man visited our hospital because of fever and general fatigue. A chest X-ray and a CT revealed pulmonary infiltration with cavities. Methicillin-resistant Staphylococcus aureus (MRSA) was cultured from his sputum and blood. Since he had no known risk factors for MRSA, he was diagnosed as necrotic pneumonia caused by community-acquired MRSA (CA-MRSA). In contrast to previously reported cases in Western countries, Panton-Valentine leukocidin (PVL) could not be detected in this case. Although inflammatory responses subsequently subsided following antibiotic treatments, lung injury progressed and he died.
