Case Report

A case of Good syndrome accompanied by a macrolide-resistant diffuse panbronchiolitis-like symptoms

ABSTRACT

The patient was a 50-year-old woman diagnosed with myasthenia gravis and thymoma who was being treated with radiochemotherapy, steroid administration, and immunosuppressive therapy after thymectomy. Approximately 5 years after the diagnosis, productive cough developed and centrilobular granular shadows in both lung fields were observed by chest contrast-enhanced computed tomography (CT). Diffuse panbronchiolitis (DPB) was ruled out by a negative HLA-B54 gene test result. Her condition was associated with hypogammaglobulinemia due to a decreased B-cell count.
Good syndrome complicated by DPB-like symptoms was diagnosed. We report this case because macrolide-resistant cases, such as that of our current patient, are rare.
