Case Report

A case of chylothorax successfully managed with paraaortic lymph node-punctured lymphangiography using ethiodol and n-butyl cyanoacrylate

ABSTRACT

A 95-year-old woman was diagnosed with chylothorax. In addition, contrast-enhanced computed tomography (CT) revealed left-sided kidney cancer and paraaortic lymphadenopathy. Conservative treatments such as fat restriction were not effective. On the sixth day of sickness, lymphangiography was performed using the embolic agent ethiodol, but ethiodol accumulated in the enlarged ilium and paraaortic lymph nodes and the reach to the chest was small, and thus, the therapeutic effect was poor. On day 35, swollen paraaortic lymph nodes were punctured under CT guidance, the lymph duct was imaged, and n-butyl cyanoacrylate solution diluted 120 times with ethiodol was injected. Subsequently, as the patient did not demonstrate an increase in pleural effusion, she was discharged from the hospital.
