Case Report

A case of nonsurgical traumatic chylothorax with leaks identified by lymphangiography

ABSTRACT

A 27-year-old woman who had suffered from dyspnea for a week was admitted to our hospital. A month earlier she had broken her upper right arm. Chest X-ray showed left pleural effusion, which was diagnosed as chylothorax. Thoracoscopy was performed with local anesthesia and revealed a gradual chylous leak from the ridges of the diaphragm. Although a chest tube was inserted, her pleural effusion did not noticeably reduce even after fat-free nutrition therapy along with a central venous catheter. Lymphangiography was performed and detected lymphatic leaks from the left thoracic duct. Octreotide with subcutaneous injection was also given for 8 days, but there was no change in the effusion. Then, thoracic duct ligation was performed via video-assisted thoracic surgery and she was discharged 23 days after admission.
