ABSTRACT

A 77-year-old man with pleural dissemination due to squamous cell lung carcinoma developed right-sided chest pain and fever following pembrolizumab administration. Narcotic analgesics and anti-inflammatory drugs were effective for symptomatic relief. Computed tomography revealed the temporary progression of multiple pleural nodules, followed by pronounced shrinkage. The clinical course of our case was consistent with pseudoprogression. Chest pain and marked inflammation were attributed to an immune response to pleural lesions. As immune checkpoint inhibitors could exhibit a non-specific response pattern, it is necessary to carefully determine the clinical effects.
