Case Report

Probable occupational hypersensitivity pneumonitis with marked eosinophilia in bronchoalveolar lavage fluid

ABSTRACT

A 39-year-old man presented with recurrent chest and back pain, fever, and cough after changing careers to the automobile parts manufacturing industry. We initially diagnosed him with pleuritis; however, on recurrence immediately after he returned to work, hypersensitivity pneumonitis was suspected. The bronchoalveolar lavage fluid showed an increase in the percentage of lymphocytes (43%) and eosinophils (35%). The environmental provocation test at his workplace was positive, and he was diagnosed as having occupational hypersensitivity pneumonitis. The case reported in the present study had a combination of the clinical features of both hypersensitivity pneumonitis and acute eosinophilic pneumonia.
