Case Report

A case of IgG4-related pleuritis with no other lesions

ABSTRACT

An 80-year-old man was referred to our hospital due to exertional dyspnea which had persisted for 3 months and left pleural effusion. He had been diagnosed with right tuberculous pleurisy at 73 years of age. We suspected IgG4-related pleuritis because the left pleural effusion was exudative and lymphocyte-dominant, and his serum IgG4 level was elevated. We performed a left pleural biopsy by thoracoscopy and observed numerous IgG4-positive plasma cells. Apart from the pleural lesion, no lesions were detected in the patient's body. We diagnosed the patient with IgG4-related pleuritis with no other organ involvement. It is necessary to consider IgG4-related disease in the differential diagnosis of unexplained pleural effusion in patients without other organ lesions.
