ABSTRACT

We present the case of a 68-year-old man who undertook janitorial services in a horse stable. He had been previously prescribed prednisolone and methotrexate for rheumatoid arthritis. In August 20XX, he presented to our hospital with symptoms of fever and fatigue. The patient's chest computed tomography showed consolidations in the left lung field; the patient had hypoxemia, which led to his hospitalization. Blood cultures from the patient revealed the presence of Streptococcus equi subsp. zooepidemicus (S. zooepidemicus). He recovered after a 14-day course of antibiotic therapy. Although bacteremia due to S. zooepidemicus is rare, it can be caused by being in contact with domestic animals such as horses. It is, therefore, important to be attentive to a patient's life history and routine to identify the causative bacteria.
