A case of lipoid pneumonia with positive PET/CT scan simulating postoperative recurrence of lung cancer

ABSTRACT

A 69-year-old man who had undergone right middle lung lobectomy for lung cancer was referred to our hospital due to the inspection of a mass located at S8 of the left lower lobe on thoracic computed tomography (CT). Although positron emission tomography (PET)/CT scan revealed the lesion with increased FDG uptake, we were unable to diagnose lung cancer with transbronchial lung biopsy. After his medication history of taking liquid paraffin as a laxative was confirmed, we suspected that he was affected by lipoid pneumonia. We performed bronchoalveolar lavage and diagnosed lipoid pneumonia based on the finding that many lipid-laden macrophages were detected in bronchoalveolar lavage fluid. It is necessary to consider the possibility of lipoid pneumonia as a differential diagnosis of lung disease showing positive on PET/CT scan.
