Case Report

A case of allergic bronchopulmonary mycosis with elevated serum CEA and PR3-ANCA levels

ABSTRACT

A 66-year-old woman complained of productive cough 2 months prior to presentation. Chest computed tomography (CT) showed a tumor shadow in the upper lobe of the right lung, and the blood test showed high carcinoembryonic antigen (CEA) and proteinase 3-antineutrophil cytoplasmic antibodies (PR3-ANCA) titers. Bronchoscopy revealed mucoid impaction in the right B3, and Penicillium corylophilum was identified in the fungal culture of the bronchial washing fluid. The patient was diagnosed with allergic bronchopulmonary mycosis, and the tumor shadow disappeared after steroid therapy. CT had shown a tumor shadow, and high serum CEA and PR3-ANCA titers were noted; therefore, distinguishing our case from those of lung cancer and granulomatosis with polyangiitis was essential.
