Case Report

A case of pulmonary cryptococcosis presenting with organizing pneumonia accompanied by respiratory failure

ABSTRACT

We present a case of pulmonary cryptococcosis causing organizing pneumonia (OP). A 70-year-old man was admitted to our hospital due to persistent cough, prompting further investigation. Chest computed tomography (CT) revealed a multifocal consolidation containing air bronchograms in both lower lung fields, suggestive of OP. Prednisolone was administered as the treatment. The serum cryptococcal antigen test was positive. We performed a bronchoscopy and obtained a transbronchial lung biopsy, which revealed the presence of fibroblasts and chronic inflammatory cells, indicative of OP. Finally, we diagnosed him with OP secondary to pulmonary cryptococcosis. Despite a poor response to steroid treatment, fluconazole was initiated, which led to an improvement in the CT findings. Pulmonary cryptococcosis presenting with OP accompanied by respiratory failure may be rapidly improved by administering combined steroid and antifungal therapies.
