A Resected Case of Well-differentiated Fetal Adenocarcinoma

A 32-year-old woman had undergone subtotal gastrectomy for gastric carcinoma on June 1993. At the time of gastrectomy, an abnormal shadow 2.5 cm in diameter was recognized in the right upper lung field on chest roentgenogram. The shadow enlarged to about 3 cm in diameter during the 10-month period following gastrectomy. The tumor was diagnosed as well-differentiated adenocarcinoma by bronchoscopic lung biopsy from right B2. Right upper lobectomy with lymph node dissection was performed in April 1994. The histological diagnosis of the resected specimen was well-differentiated fetal adenocarcinoma (WDFA). The postoperative pathological staging revealed p-T1N0M0 Stage I. Histopathologically, the tumor simulated fetal lung and contained well-ordered glandular, full growing epithelium and lacked sarcomatous features. Although reported cases treated by lobectomy have demonstrated a good clinical course, our patient whose tumor showed invasion of a vessel had multiple pulmonary metastases 18 months after the operation.
