Giant Atypical Carcinoid Tumor of the Lung

Case: A 52-year-old woman was hospitalized with a solitary mass shadow on chest X-ray film. Chest CT scan showed a 10×10 cm sized tumor in the right upper lobe, compressing superior vena cava. Transbronchial biopsy failed to obtain a definitive diagnosis, then percutaneous needle biopsy was performed. Microscopic examination of a specimen demonstrated tumor cells with mitosis and positive staining for neuron-specific-enolase. From these findings, pathological diagnosis of atypical carcinoid was made. An induction therapy with concurrent chemoradiation was performed, resulting in approximately 80% reduction of tumor size. Thereafter right upper lobectomy was performed, however, remaining of tumor cells was confirmed pathologically in a tissue specimen obtained from parietal pleura. The patient has been in good condition without any signs of recurrence more than 6 months after the operation.
