Three Surgical Cases of Bronchogenic Carcinoma with Coexisting Active Pulmonary Tuberculosis

Department of Surgery, Takamatsu National Hospital

Recently, both lung cancer and tuberculosis have been increasing. The frequency of patients suffering both of these diseases concurrently is high and will probably increase in the future. We reported three patients with lung cancer accompanied by active pulmonary tuberculosis who successfully underwent operations for lung cancer. Case 1 was a 71 year-old man with stage IB adenocarcinoma, still alive at 9 years after right upper lobectomy. Case 2 was a 44 year-old woman with stage IIB adenoid cystic carcinoma, presently alive at 6 years after right middle and lower lobectomy and radiation therapy. Case 3 was a 79 year-old man with stage IA squamous cell carcinoma, surviving for 10 months after left upper lobectomy. Case 1 postoperatively showed the complication of active pulmonary tuberculosis. Cases 2 and 3 had received chemotherapy against tuberculosis for 1 month prior to operation, and both were operated when sputa tested negative for tuberculosis. All three cases made progress without any complications and in all cases the pulmonary tuberculosis was cured by postoperative treatment. Even in patients with concurrent lung cancer and tuberculosis, conventional treatment can be effective and good prognosis can be anticipated for both diseases if they are diagnosed at an early stage. In patients with abnormal shadows in the chest, not only lung cancer but also tuberculosis, or both concurrently, should be taken into consideration in the diagnostic procedure.
