Background: We describe a case of peripheral adenoid cystic carcinoma of the lung with high serum CEA value, with reference to 10 cases reported in the Japanese literature. Case: A 66-year-old woman was admitted with an abnormal shadow in the chest X-ray, accompanied by high serum CEA value. Chest CT film revealed two peripheral mass shadows in the right middle lobe of the lung. Right middle lobectomy was performed and histological examination confirmed adenoid cystic carcinoma of the peripheral lung with immunohistochemically positive CEA. After the operation the CEA value of the patient decreased from the preoperative value. Conclusion: Peripheral adenoid cystic carcinoma of the lung is uncommon. In particular, cases with high CEA value, as seen in the presented case, are rare.
