A Case Report of Lung Cancer with Pulmonary Cryptococcosis in the Contralateral Lung

Background: Pulmonary cryptococcosis is relatively rare, but it is sometimes difficult to differentiate from lung cancer due to its appearance in X-ray examination. The preoperative exact diagnosis is important to perform suitable treatment. Case: A 46-year-old woman was admitted with an abnormal chest shadow. Chest CT showed multiple nodular and consolidated shadows with an air-bronchogram in the right S6 and a mass shadow with pleural indentation in the left S8. The lesions were diagnosed by TBLB as pulmonary cryptococcosis in the right S6 and adenocarcinoma in the left S8. Video assisted left lower lobectomy was performed, and an antifungal agent was administered preoperatively and continued after operation. Her postoperative course was uneventful. Conclusion: In this case, both lesions were diagnosed preoperatively by TBLB, and she underwent minimally invasive surgery and given effective medication. In cases which are accompanied with multiple shadows including lung cancer, it is important to differentiate from pulmonary metastasis to avoid exessive surgery and losing a chance for suitable treatment.
