The Prognostic Significance of Lymphatic Vessel and Blood Vessel Invasion in Peripherally Located Adenocarcinoma of the Lung

Objective: The relationship between prognosis and lymphatic vessel and blood vessel invasion in peripherally located adenocarcinoma of the lung was examined. Methods: Between January 1989 and December 1999, 344 patients with peripherally located adenocarcinoma of the lung were resected in our hospital. In these cases, outcome in relation to lymphatic vessel and blood vessel invasion was examined clinicopathologically. Results: The median follow-up time was 42 months. The overall 5-year survival rate was 68.2%. The 5-year survival rate was 96% in ly0v0 cases, 75% in ly0v1 cases, 75% in ly1v0 cases, 65% in ly1v1 cases, and 37% in ly2, 3 and/or v2, 3 (lyv 2-3) cases, respectively. The 5-year survival rate of stage IA disease was 98% in ly0v0 cases, 100% in ly0v1 cases, 96% in ly1v0 cases, 84% in ly1v1 cases, and 68% in lyv 2-3 cases, respectively. Obviously the outcome of ly0v0 cases was good. There was no difference in prognosis by the tumor diameter in ly0v0 cases. The ratio of patients with stage I disease was 97% in ly0v0 cases, 82% in ly0v1 cases, 62% in ly1v0 cases, 52% in ly1v1 cases, and 25% in lyv 2-3 cases, respectively. Most of the ly0v0 cases were stage I disease. The ly0v0 cases comprised 51% of stage IA disease, 23% of stage IB disease, and almost none were of an advanced stage. In 15 patients with limited operation in ly0v0 cases, the 5-year survival rate was 100%. Conclusions: It is concluded that lymphatic vessel and blood vessel invasion are significant prognostic factors.
