Clinical Examination of Acute Exacerbation of Idiopathic Interstitial Pneumonia (IIP) Combined With Lung Cancer After Anti-cancer Treatment

Objective: To know the risk factors of acute exacerbation of idiopathic interstitial pneumonia (IIP) after anti-cancer treatment in patients with lung cancer combined with IIP, we examined the clinical features of lung cancer patients combined with IIP who received anti-cancer treatment. Methods: Forty-five patients with lung cancer and IIP who were admitted in our hospital from 1991 to 2000 were examined. Results: Acute exacerbation of IIP after anti-cancer treatment occurred in 7 patients (15.6%) out of 45 patients who received chemotherapy, radiotherapy, chemoradiotherapy or surgical resection. The incidence of acute exacerbation of IIP was significantly higher in the cases with poor PS, elevated CRP, elevated LDH, increased WBC, decreased PaO2 or decreased %VC as compared with the cases with normal values. Therefore, these were considered the risk factors for the acute exacerbation of IIP. The cases with multiple risk factors were more susceptible, the incidence of the acute exacerbation of IIP being 100% in cases with 4 or more risk factors, while it was 2.8% in the cases with 2 or less risk factors. Conclusion: Lung cancer patients with IIP who have multiple risk factors should not receive anti-cancer treatment, because they are more susceptible to the acute exacerbation of IIP.
