TTF-1 Immunostaining in the Diagnosis of Gastric Metastasis From Lung Adenocarcinoma

Background. Although gastric metastases from lung cancer are considered to be uncommon, the distinction of a primary gastric carcinoma from a metastatic lesion of lung cancer is often difficult because of overlapping pathologic features. Recently the antibody to thyroid transcription factor-1 (TTF-1) has been shown to be a fairly specific marker for lung cancer. We report two cases of gastric metastasis from lung adenocarcinoma with positive immunostaning with TTF-1. Case. Case 1 was a 56-year-old woman who was referred to our hospital with an abnormal shadow on a chest X-ray film. The tumor was diagnosed as adenocarcinoma (cT1N0M0 stage IA) . Nine months after curative resection, she developed epigastralgia, and gastrointestinal endoscopy was performed. Case 2 was a 52-year-old man referred to our hospital with malignant pleural effusion. The clinical stage was IIIB (cT4N2M0) . After a second systemic course of chemotherapy with carboplatin and paclitaxel, he complained of epigastric discomfort. Gastrointestinal endoscopy was performed. The histological specimens of both cases were immunostained with a monoclonal antibody to TTF-1. Conclusion. Two cases of lung adenocarcinoma with metastasis to the stomach are reported, in each of which immunostaning for TTF-1 was useful in obtaining an accurate diagnosis.
