Efficacy of Annual Repetition on the Results of Lung Cancer Screening

Objective. We evaluated the efficacy of annual repetition on the results of lung cancer screening. Methods. A total of 48630 participants aged 40 to 79 years, who underwent lung cancer screening with chest radiography and sputum cytology from 1998 to 2004, were studied. We classified the participants into the initial screening group and the annual repeated screening group, in order to compare the results of screening and the clinical characteristics of detected lung cancer cases. Results. There were 16222 participants in the initial screening group and 23669 in the annual repeated screening group. The distribution of age and gender did not show statistically significant differences. The rate of further detailed examinations needed was 6.3% in the initial screening group and 2.6% in the annual repeated screening group, showing a statistically significant difference. The lung cancer detection rate was 111.0/100000 in the initial screening group and 80.3/100000 in the annual repeated screening group. The mean diameter of the detected tumors was 3.5±1.9 cm in the initial screening group and 2.4±1.5 cm in the annual repeated screening group, the rate of clinical Stage IA lung cancer cases was 27.8% in the initial screening group and 68.4% in the annual repeated screening group, showing statistically significant differences. The surgical resection rate and the complete resection rate in the annual repeated screening group were higher than those of the initial screening group. Conclusion. This study revealed that the annual repetition of lung cancer screening did not increase the lung cancer detection rate but was effective in detecting early lung cancer.
