Extrapleural Pneumonectomy for Malignant Pleural Mesothelioma

Objective. We assessed the outcome of surgical treatment for malignant pleural mesothelioma (MPM), to clearly whether extrapleural pneumonectomy (EPP) is effective treatment. Patients. The subjects were 37 patients with MPM treated at our hospital from 1993 to 2004. Results. We analyzed 10 patients with MPM who underwent EPP. The 10 men had a mean age of 61.5 years old. Histologically, 3 of them were epithelial type, 1 was biphasic type, 6 were desmoplastic type. According to the staging of International Mesothelioma Interest Group, 9 patients had stage I disease. 1 had stage III. One patient (stage III) had irradiation with chemotherapy and one patient had irradiation only after operation. Patients with EPP and patients without surgery had median survivals of 22 months and 12 months, 1-year survival rates of 67.5% and 50.0% respectively. In particular stage I case had a 3-year survival rate of 85.7%. Conclusion. Extrapleural pneumonectomy can be effective treatment for early stage malignant pleural mesothelioma.
