A Case of Squamous Cell Carcinoma Accompanied with Carcinoid in Which Positron Emission Tomography Scan Was Useful for Preoperative Staging of Lung Cancer

Background. Preoperative staging of lung cancer is often based on chest computed tomography (CT) scan. However, it has been recognized that combination with positron emission tomography (PET) scan can also be effective. Case. We describe a 73-year-old man in whom an abnormal shadow at left upper lobe was found on chest radiography, and in whom lung squamous cell carcinoma was diagnosed by transbronchial lung biopsy. A chest CT scan showed a solid nodule 19 mm in diameter, which was considered to be the primary lesion in the left peripheral S3 segment, and another nodule 18 mm in diameter in the adjacent B1+2 bronchus. Therefore, lung cancer with metastasis to hilar lymph node was diagnosed and it was classified as clinical stage IIA. However the PET scan prior to surgery demonstrated accumulation only in the S3 segment tumor. We performed left upper lobectomy with lymph node dissection. Pathological findings showed that the nodule which seemed to be a metastatic lymph node was typical carcinoid. We finally diagnosed squamous cell carcinoma accompanied with carcinoid with no lymph node metastasis and pathologically classified it as stage IA respectively. Conclusion. Combined evaluation using CT and PET scan for assessment of nodal status can be reliable and informative. In this case, PET scan was useful for preoperative staging.
