A Case of Solitary Fibrous Tumor of the Pleura with Initial Onset of Hemothorax

Background. Solitary fibrous tumor of the pleura (SFTP) is an uncommon neoplasm arising from the submesothelial tissue. We report a case of malignant SFTP with initial massive hemothorax. Case. A 57-year-old man who was receiving warfarin for a cerebral embolism visited our hospital with a complaint of cough and dyspnea. He was given a diagnosis of left spontaneous massive hemothorax, and we performed chest drainage. However, he did not improve with conservative treatment, and therefore we performed thoracotomy. A large necrotic tumor which had ruptured was noted in the left lower lobe intraoperatively, and therefore a lobectomy was performed in addition to the removal of the clot. No residual lesion was found macroscopically within the thoracic cavity. A pathological examination revealed the tumor to be a malignant SFTP, which had invaded the lung and caused extensive hemorrhagic necrosis. The tumor recurred locoregionally and grew rapidly over 3 months postoperatively, and the patient died of respiratory and circulatory failure due to tumor compression shortly afterwards. Conclusion. This case was an unusual clinical presentation of a rare pleural tumor. It is very rare that SFTP features initial hemothorax. The prognosis of this type of tumor after removal is usually not poor, but the present case had a fatal outcome due to early postoperative recurrence.
