A Case of Solitary Fibrous Tumor of the Pleura with Pulmonary Metastasis 21 Years After Initial Surgery

Background. Pulmonary metastasis of solitary fibrous tumor of the pleura (SFTP) after complete surgical resection is rare. Case. A 75-year-old woman underwent resection of SFTP originating from the visceral pleura of the right upper lobe of the lung in February 1984, and re-excision of locally recurrent SFTP in January 1994. Microscopically the primary and recurrent tumors showed no evidence of malignancy. A postoperative follow-up chest X-ray film in April 2005 revealed an abnormal shadow in the left lung. As repeated chest X-ray films demonstrated the lesion to be enlarging, the patient was referred to our hospital in January 2007. A computed tomographic (CT) scan and magnetic resonance image (MRI) showed a well-circumscribed, heterogeneous mass shadow in the left lower lobe of the lung, suggesting intrapulmonary SFTP. The lesion was removed by wedge resection of the left lower lobe through a thoracotomy. The histological appearances of the lesion were similar to those of the previously excised lesions. However, an increase in the number of mitotic cells and on the MIB-1 index was seen in the left lung lesion. The diagnosis of pulmonary metastasis of benign SFTP with malignant transformation was made. The patient is doing well without recurrence 4 years after operation. Conclusion. We present a rare case of benign SFTP which metastasized 21 years after initial surgery with histological evidence of malignant transformation. Our case indicates that a careful long-term follow-up is necessary for patients with SFTP.
