A Case of Tracheoesophageal Cyst in the Posterior Mediastinum

Background. Tracheoesophageal cyst of the mediastinum is an extremely rare disease, with only 6 cases reported in the world, including the present case. Case. A 43-year-old woman had an abnormal shadow in the left lower lung field on a chest X-ray film. A chest CT scan revealed a spherical tumor-like shadow with a distinct margin and low density near the descending aorta in the posterior mediastinum. On T1-weighted MRI the mass showed homogeneous hyperintensity, and on T2-weighted MRI the mass showed marked hyperintensity. Thoracotomy was performed for excision of the posterior mediastinal tumor. The tumor was 6×3×3 cm unilocular cyst, containing a gelatinous substance. It had no communication with the esophagus or bronchus. Histologically, it was lined by ciliated columnar epithelium in some areas and by squamous epithelium in others, and had bronchial glands, cartilage plates, esophageal glands, and a large amount of muscle tissue with several layers of smooth muscle tissue in its wall. A diagnosis of tracheoesophageal cyst of the posterior mediastinum was established on the basis of the combined features of bronchogenic and esophageal cyst. Conclusion. We report an extremely rare case of tracheoesophageal cyst in the posterior mediastinum and review the relevant literature.
