An Autopsied Large Cell Neuroendocrine Carcinoma of the Right Hilum Treated with Chemotherapy

Background. Although pulmonary large cell neuroendocrine carcinoma (LCNEC) has usually appeared as a peripheral mass, reports on LCNEC of the hilum treated with chemotherapy are limited. Case. A 73-year-old man with fever and cough was referred to our hospital in July 2003. Chest radiography showed abnormalities in the bilateral hila. Cytology by fiberoptic bronchoscopy demonstrated poorly differentiated squamous cell carcinoma. The clinical stage was T4N3M0 (stage IIIB). The patient received 3 cycles of chemotherapy with cisplatin plus paclitaxel, which achieved a partial response. However, lung cancer metastasis to the lung and the liver were noted in late November 2003. He died in early May 2004, despite further chemotherapy (2 cycles of cisplatin plus vinorelbine, 1 cycle of irinotecan monotherapy, and 1 cycle of amrubicin monotherapy). An autopsy revealed LCNEC of the right hilum. Conclusion. We encountered a partial response case of LCNEC of the right hilum treated with chemotherapy.
