Background. Pulmonary aspergillosis in lung cancer rarely occurs simultaneously, although a few cases have been reported in Japan. This report concerns a case of vacuous lung cancer, in which Aspergillus fungal components was detected in the resected specimen. Case. The chest X-ray of a 76-year-old man revealed a vacuous round tumor in the lower lobe of his left lung. The thin-section CT image revealed thick cavity walls, but the thickness of the walls was irregular and partitioned. A lower lobectomy of the left lung was performed. The histological diagnosis of the tumor unveiled poorly differentiated adenocarcinoma (stage IIB, pT3N0M0), which included Aspergillus fungal components. Conclusion. Lung cancer involving aspergillosis is rare and cavities are detected in many of such cases. In this case, we concluded that the Aspergillus infections were can be caused by local immunodeficiency, induced by lung cancer. The cavities in this case were formed because of several factors.
