A Case of Recurrent Lung Adenocarcinoma Associated with Paraneoplastic Dermatoses

Background. Paraneoplastic dermatoses are nonspecific disorders of the skin that are associated with internal malignancy. Cutaneous manifestations are often found before the diagnosis of malignancy or recurrence of cancer. We report a case of a paraneoplastic skin disorder associated with recurrence of lung cancer. Case. A 63-year-old man was referred to the Department of Thoracic Surgery of our hospital because of an abnormal shadow detected on chest CT. He previously underwent surgery for lung cancer at age 58, but was lost to follow up 2 years later. On his visit to a dermatologist, he presented with cutaneous prurigo chronica multiformis which did not resolve with commonly used medication. Biopsy under bronchoscopic guidance revealed the recurrence of lung cancer. Systemic chemotherapy was thus administered, resulting in tumor regression and the disappearance of cutaneous manifestations. During follow-up, the skin lesions recurred, and chest CT scan revealed tumor regrowth. Tumor regression and reduction of cutaneous manifestations were achieved with additional chemotherapy and radiotherapy. Conclusion. We report a case of recurrent lung cancer associated with paraneoplastic dermatoses. The skin lesions tended to recur with the growth of the lung tumor. As skin lesions may be indicative of cancer recurrence, a careful diagnostic work up is necessary when patients with dermatoses do not respond to therapy.
