Background. We herein report a case of lung metastasis of sigmoid colon cancer presenting as a thin-walled cavity. Case. The patient was a 55-year-old female receiving follow-up after undergoing surgical resection for sigmoid colon cancer (SE, N0, P1, Stage IV) followed by seven months of chemotherapy with BV plus mFOLFOX6. A preoperative chest CT scan revealed a 8-mm thin-walled cavity in the left S6 that was initially diagnosed as a lung cyst. The cavity gradually increased in size to a diameter of 20 mm, with thickening of the cavity wall, three years after surgery. Thoracoscopic wedge resection was subsequently performed, and the lesion was diagnosed as metastasis of sigmoid colon cancer. Conclusions. Careful follow-up and proactive efforts to obtain a definitive diagnosis are necessary in patients with expanding thin-walled cavities.
