Introduction. Primary pulmonary synovial sarcoma is extremely rare. We herein report a case of primary pulmonary synovial sarcoma with contriving using the literature. Patient. A 53-year-old man was admitted to our hospital for the treatment of a tumor of the left lung. Computed tomography revealed a 48-mm tumor in the left upper lobe of lung. We diagnosed the tumor as primary lung cancer and performed left upper segmentectomy. The tumor showed proliferation of spindle cells that were stained positively for EMA, keratin, bcl-2, and TLE1. We ultimately diagnosed the lesion as primary monophasic pulmonary synovial sarcoma. Discussion. The preoperative diagnosis of synovial sarcoma is difficult. Early surgical treatment is important for improving the patients' overall survival.
