Background. Combined large cell neuroendocrine carcinoma (combined LCNEC) is defined as LCNEC combined with adenocarcinoma, squamous cell carcinoma, or pleomorphic carcinoma. Combined LCNEC can be combined with any non-small cell non-neuroendocrine cancer. Case. We herein report the case of an 85-year-old man with combined pulmonary LCNEC and basaloid squamous cell carcinoma (BSCC). He was admitted to our hospital due to the presence of a mass with an irregular shape in the right upper lobe of the lung on positron-emission tomography (PET)-CT. A biopsy of the bronchoscopy was performed, and the pathology suggested small cell lung cancer (SCLC). We observed no brain metastases or other distant metastases. He was treated with thoracoscopic radical surgery for upper right lobe. The postoperative pathology showed combined pulmonary LCNEC with BSCC. The tumor that had been diagnosed with SCLC based on the transbronchial biopsy was BSCC. Immunohistostaining contributed to the diagnosis. Conclusion. This is a very rare case of combined pulmonary LCNEC with BSCC that was diagnosed by a postoperative pathological analysis.
