A Case of Fibrosing Mediastinitis That Regressed Spontaneously After a Thoracoscopic Biopsy

Background. Fibrosing mediastinitis is a rare disease that forms a tumor-like structure consisting of fibroblast cells, collagen fibrils and chronic inflammatory cells. Case. A 66-year-old man visited our hospital with a cough. Chest computed tomography showed a solid tumor from the aortic arch to the left hilum of the lung. Fluorodeoxyglucose strongly accumulated in the tumor. A thoracoscopic biopsy was performed for a definitive diagnosis. The tumor was hard and adhered strongly to the left upper lobe, making it difficult to sample. The tissue volume was also small, so it was difficult to distinguish between a malignant tumor and inflammatory lesion. After the patient was referred to another hospital, the tumor had visibly decreased in size, so an additional biopsy was not performed. These findings led us to a diagnosis of fibrosing mediastinitis. In the two years without treatment, the tumor mass has not enlarged. Conclusion. When a nonspecific tumor is found in the mediastinum, the possibility of fibrosing mediastinitis should thus be considered when administering treatment.
