Background. There are still few reports of tracheal sleeve resection and reconstruction in elderly patients. Case. An 84-year-old woman was referred to our hospital with a diagnosis of tracheal adenoid cystic carcinoma, which was revealed by a transbronchial biopsy. Chest computed tomography (CT) revealed a tumor in the anterior wall of the thoracic trachea. We performed tracheal sleeve resection and reconstruction through median sternotomy. The postoperative course was uneventful. The patient was discharged from the hospital and allowed to return home 33 days after the surgery. Conclusion. A female octogenarian patient with tracheal adenoid cystic carcinoma was successfully treated with tracheal sleeve resection and has been well for 12 months without any signs of recurrence.
