The Effect of Pulmonary Metastasectomy for Metastases from Osteosarcoma Or Soft Tissue Sarcoma

Objective. Pulmonary metastases from osteosarcoma or soft tissue sarcoma occur frequently. This study aimed to investigate the postoperative prognosis of patients that underwent pulmonary metastasectomy and prognostic factors for such patients. Methods. From 2003 to 2015, 28 consecutive patients with pulmonary metastases derived from osteosarcoma or soft tissue sarcoma underwent resection at our institution. Their clinical records and survival were retrospectively reviewed. Univariate (log-rank) and multivariate Cox regression analyses were carried out to identify the prognostic factors related to the overall survival. The survival rates were estimated using the Kaplan-Meier method. Results. The patients' median age was 61 years (range: from 13 to 82 years), and 17 patients were male. The primary tumor was histologically classified as osteosarcoma in 11 patients, malignant fibrous histiocytoma in 12 patients, synovial sarcoma in 3 patients, and liposarcoma in 2 patients. The tumor size and number of resected lesions ranged from 0.1-8.0 cm and 1-7 lesions, respectively. No morbidity or mortality occurred after metastasectomy. The overall 5-year survival rate was 47.9%, and a multivariate analysis identified the age at the time of the operation and the disease-free interval as significant prognostic factors. Conclusion. Pulmonary metastasectomy for metastases from osteosarcoma or soft tissue sarcoma may improve the prognosis, particularly in cases involving younger patients or smaller tumors. Further investigations in the future are necessary.
