Background. The number of patients with lung cancer coexistent with pulmonary non-tuberculous mycobacteriosis (NTM) has increased in recent years with the increasing incidence of pulmonary NTM and lung cancer. We herein report a case of metachronous multiple lung cancers coexistent with pulmonary NTM. Case. A 72-year-old woman underwent right upper lobectomy with lymph node dissection because of lung adenocarcinoma coexistent with lung NTM. Two years later, she was admitted to our hospital because of cough. Chest computed tomography showed a growing tendency of ground glass opacities and three new nodules. Left S6 segmentectomy was performed because three nodules showed a growing tendency even after anti-tuberculous chemotherapy for three months. Histologically, the nodules were diagnosed as adenocarcinoma in situ. Conclusion. The development of lung cancer should be considered when lesions of lung NTM continue to grow despite appropriate chemotherapy.
