Background. Small cell lung cancer metastasizes to the whole body, but thyroid metastasis is relatively uncommon. Case. A 66-year-old man was diagnosed with small cell lung cancer (cT4N3M0, LD) and with superior vena cava syndrome in October X. A partial response was obtained by chemotherapy and radiotherapy from the ipsilateral hilum to the mediastinum. In March X+1, the tumor relapsed at the right supraclavicular lymph nodes and was treated with combination therapy, which resulted in a complete response. In March X+2, solitary brain metastasis developed and was treated by cyber knife. In September X+2, a nodule was found in the right lobe of the thyroid gland without metastasis in any other organ. The thyroid nodule proved to be metastasis from small cell lung cancer after lobectomy. At three years after the diagnosis of small cell lung cancer, there has been no relapse. Conclusion. Malignant metastasis to the thyroid gland are not uncommon in autopsy cases, but few cases are reported in living patients. We reported a case of solitary metastasis to the thyroid gland from small cell lung cancer intra vitam.
