Background. Basaloid squamous cell carcinoma (BSC) is a rare variant of squamous cell carcinoma with a poor prognosis. Case. The patient was a 76-year-old man in whom a left lung nodule was found on computed tomography (CT) following colorectal cancer. The nodule gradually increased in size and a bronchoscopic biopsy was performed from the branch (B1+2c) of the left upper bronchus. The preoperative diagnosis was squamous cell carcinoma. Left upper lobectomy and lymphadenectomy (ND2a-2) were performed. The final diagnosis was basaloid squamous cell carcinoma T1bN0M0, pl0, V0, Ly0, br (-), pm0, Stage IA2. The patient is currently alive without relapse at more than 5 years after surgery. Conclusion. BSC has a poor prognosis and careful follow-up is necessary for patients with this disease.
