A Case of Primary Pulmonary Classical Hodgkin Lymphoma

Background. Pulmonary Hodgkin lymphoma is a rare tumor, and its preoperative diagnosis is difficult. Case. A 39-year-old woman showed an abnormality on chest radiography during the preoperative examination of appendicitis. Computed tomography (CT) of the chest revealed a pulmonary mass 5 cm in diameter in the left upper lobe that reached the pericardium. Transbronchial biopsies (TBBs) revealed a malignant appearance of unknown histology. Left upper lobectomy and mediastinal lymph node dissection were subsequently performed. Microscopic findings showed malignant cells containing diffuse Hodgkin cell and multinucleated Reed-Sternberg cells. Immunohistochemical findings revealed that the tumor cells were positive for anti-CD30 and anti-CD15 antibody. Therefore, this tumor was diagnosed as classic Hodgkin lymphoma, nodular sclerosis type, with a pulmonary origin, as extrapulmonary disease was not present. Conclusion. It is important to make an accurate diagnosis of pulmonary malignant lymphoma in order to select the proper treatment.
