Background. Lung tumors with rhabdoid phenotype were included in the 1999 WHO classification of lung and pleural tumors as a special type of large-cell carcinoma of the lung, but in the 2021 WHO classification of lung tumors, a rhabdoid phenotype is now regarded as a cytologic feature, and lung tumors with the rhabdoid phenotype have a poor prognosis and do not belong to a specific histological subtype. There are few reports on such lesions, and no treatment has yet been established. Case. A 72-year-old female underwent a biopsy due to swelling of the right mandibular gingiva. Subsequently, an abnormal chest shadow was pointed out on chest radiograph, so a transbronchial lung biopsy was performed, and based on the results of the right mandibular gingival biopsy, we diagnosed her with a lung tumor with the rhabdoid phenotype that had metastasized to the right mandibular gingiva. After determining the diagnosis, the patient's general condition worsened, so radiation was only administered to the oral lesions, but her condition suddenly deteriorated, and she died 30 days after the diagnosis. Conclusion. We experienced a rare case of a lung tumor with the rhabdoid phenotype that metastasized to the gingiva. Lung tumors with the rhabdoid phenotype are aggressive and highly malignant, and their treatment has not been adequately studied, so no standard treatment has yet been established. Therefore, the early detection and establishment of effective treatment are desired.
