Abstract We prospectively evaluated the performance of the Verigene System with the Verigene Gram-positive Blood Culture Test (BC-GP) and Verigene Gram-negative Blood Culture Test (BC-GN) in positive blood culture bottles between July 2014 and March 2016. The concordance rates were evaluated with phenotypic identification methods and conventional molecular examinations. In total, we evaluated 272 positive blood cultures bottles (282 strains), and the median duration between the initiation of the blood culture incubation and the reporting of the Verigene system result was 22.7 h (interquartile range 19.1-29.2 h). For the evaluation with the BC-GP, the concordance rates were 99.0% (103/104) for bacterial identifications and 100% (53/53) for drug resistance. For the evaluation with the BC-GN, the concordance rate was 97.8% (174/178) for bacterial identifications, and antimicrobial resistance genes were identified in 15 bottles (blaCTX-M: 13, blaIMP: 2). For additional evaluations of the BC-GN, we collected 96 cefotaxime-non-susceptible strains (Escherichia coli: 82, Klebsiella pneumoniae: 7, Klebsiella oxytoca: 3, Peroteus spp.: 4) that had been isolated from patients with bacteremia at 6 facilities, and resistance genes were identified using the Verigene system with the BC-GN in 88 strains (91.7%). All of the resistance genes were confirmed with conventional molecular examinations. In our current study, we confirmed that Verigene system could promptly identify bacteria and resistance genes directly from positive blood culture bottles.
