Abstract Matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS) is a cost-saving, rapid, easy and high-quality method compared with the conventional methods. In Kyushu University Hospital, VITEK MS (one of the models of MALDI-TOF MS from bioMerieux Japan) was introduced in January 2013. This is the first report on the cost of microbial tests and identification time using MALDI-TOF MS in Japan. We compared procedure time, identification time and costs in 2011 and 2015 or before and after VITEK MS introduction. The numbers of identification of aerobes, anaerobes and yeasts were counted. While the specimens in 2011 and 2015 were almost the same, 25,178 and 25,994, respectively, the identification number in 2011 and 2015 were 3,425 and 26,951, respectively. We determined the reagent cost, expense cost (reagent cost plus examination cost), total cost (expense cost plus maintenance cost) and identification time (examination time plus reaction time). After introduction of VITEK MS, the usage of conventional identification kits decreased by 62.6%. However, the identification by VITEK MS increased up to 26,112. The identification time per strain decreased from 848.9 to 45.3 minutes (about 13.4 hours). Reagent and expense costs slightly decreased, but the total cost increased. Introducing VITEK MS, we've become able to identify a larger number of specimens more rapidly and precisely, but it did not lead to cost savings when considering maintenance cost. The merit of introduction on VITEK MS is the improvement of accuracy, which enables us to select appropriate antibiotics and to launch effective infection control.
